Prepare for Your Visit
For your convenience, we offer two online forms to download and print. We encourage you to fill out both forms and bring them with you to your visit -- this includes a Medical History Review and a New Patient Registration form.
A list of your medications is also important to remember to bring with you – your doctor will review your list for potential side effects that involve the eye.
Present all insurance identification cards prior to receiving service, this includes both vision and medical plans. Unlike your primary care doctor who only bills your insurance company based upon your health, optometrists have the ability to not only care for your VISION, but also the HEALTH of the eye.

Release of Eye Care Records from a Previous Provider
If you are transferring care to our office from another eye care provider and wish to have your records sent to our office, please stop by our office prior to your appointment and sign a records release form.
Payment is required in full at the time of service. We accept payments from most insurance plans and all major credit cards. We offer budget-friendly as well as high-end glasses in the optical department and the option of Care Credit Financing to accommodate you.
